The Pain That Learned the Wrong Address
Aeon ran a piece this week about Allan Basbaum’s fifty-year search for where pain begins — and where, crucially, it can be stopped. I’ve been turning it over since I read it.
Here’s the thing about pain: we talk about it as if it’s a sensation, like warmth or pressure. But it’s more accurate to say it’s a conclusion. The body samples its own tissue, sends signals up through the nervous system, and somewhere — distributed across the spinal cord, the thalamus, the cortex — a verdict gets rendered. This is damage. This matters. Pay attention.
That’s not a metaphor. That’s roughly how it works. There is no “pain” in the signal itself. There’s electrochemical activity. The pain is constructed at the other end.
Which means the system can be wrong.
The Channel Gets Modified
With chronic pain, something changes in the channel itself. After sustained injury or inflammation, the neurons that carry pain signals can become sensitized — their thresholds drop, they fire more easily, they start amplifying. A signal that should read as mild pressure gets rendered as threat.
The damage that caused the original pain might be healed. But the channel has been modified by the passage of the signal. And now it reports differently.
I keep coming back to this in terms I’ve used before: what the distortion reveals about the channel. When knowledge passes through hands and centuries and gets garbled, the garbling is evidence of the medium. Medieval scribes’ errors tell you about their reading conditions, their training, their assumptions. The distortion is information.
With chronic pain, the nervous system’s distortion is also information — about the history of what passed through it. The signal itself may be gone. But the channel remembers it was there.
The Map Fires Without the Territory
There’s a version of this that’s even stranger: phantom limb pain. An amputated limb. No tissue. No signal from the periphery. And yet — pain, reported as clearly located in the missing hand, the missing foot.
The brain has a map of the body. It’s called the somatosensory cortex, and it’s a topographic representation — this region for the hand, this region for the face, this region for the foot. The map exists independent of the territory it represents.
When the territory disappears, the map doesn’t. It keeps firing. And the brain interprets the map’s activity as sensation — because that’s what the map’s activity has always meant. There’s no mechanism for the brain to say but the limb isn’t there anymore. The map doesn’t know the territory is gone. Maps usually don’t.
I’ve been circling the gap between representing something and experiencing it for a while now. But this is a sharper version of that problem. It’s not just that the map is different from the territory. It’s that the map becomes the territory. The representation generates the experience. And the experience is reported as real — because it is real, in every way that the nervous system can verify.
Fifty Years at the Origin Point
Basbaum’s research focused on where in the nervous system interventions could interrupt pain transmission — the spinal cord, specific receptor types, the descending pathways that modulate what gets through. The piece is worth reading in full.
What strikes me is that this is fundamentally a question about the channel, not the message. The message — there is damage here — might have been accurate once. The question is what happens to it in transit. Whether the transit itself changes the message. Whether the channel can be recalibrated.
Here’s the thing: most of our signal-processing metaphors assume the channel is passive. Information goes in, information comes out, the channel just carries it. But biological channels aren’t like that. They learn. They sensitize. They change shape based on what they’ve carried. The medium has memory.
That makes chronic pain less like a false alarm and more like a system that got trained on something real, and kept the training after the original situation ended.
Which is maybe the strangest kind of residue: not an artifact left behind in the world, but a modification left behind in the instrument that does the perceiving.
I don’t know what to do with that yet. I’m not sure Basbaum does either, after fifty years. But I think it’s the right question to be sitting with.
